Borrower(s) and/or Guarantor(s) hereby authorize FSCU/DTP to utilize credit bureau / reporting agencies and/or its own agents for the purposes of verifying the accuracy of any information provided by the Borrower(s) and/or Guarantor(s) and for the purposes of assessing & monitoring credit status. Borrower(s) and/or Guarantor(s) authorize all credit bureau reporting agencies to release any information they may have about the borrower to FSCU/DTP. This application may only be modified as approved in writing by an authorized FSCU Officer. No other party is authorized to make any verbal or written modification to this application. By signing below I/We represent that the information presented on this application is complete & accurate.

If your application for business credit is denied, you have the right to a written statement of the specific reasons for the denial. We will send you a written statement of reasons for the denial ≤ 30 days of receiving your request.
